Vaccination Verification for In-person Attendance – High Holy Days 5782/2021

Please fill out your name and e-mail address, click the upload box, and select the image of your vaccination card that is stored on your device. You can upload a photo or scanned copy of your card. (If you don’t have a stored image of the card and are on a mobile device, you’ll be given the option of taking a picture of your card.) Then click Submit.

If you prefer not to use this form, you can:

• Take a picture of the vaccination card, print the picture, and mail it to the synagogue office at 350 Roxbury Road, Stamford, CT 06902, with the individual’s name and e-mail address.
• Take a picture of the vaccination card, attach the picture file to an e-mail, and send it to vax@tbe.org with the individual’s name and address in the body of the e-mail.
• Stop by the synagogue office where a copy of your vaccination card will be taken.
   

Remember, you will not receive your ticket for the High Holy Days unless you have submitted proof of vaccination. We will delete all documentation once the High Holy Days are over.
